How to Access Services

The following is some simple information about the process of applying for UnitingCare Ageing services.

Residential Care

If you wish to apply for residential care (nursing home or hostel) in UnitingCare Ageing you need to provide us with a COPY of the following documentation:

1.      An up to date Aged Care Assessment Team (ACAT) assessment (see below for more information)

2.      A completed application form from the Department of Health & Ageing 5 step pack (Click here to download application form). The complete 5 step pack is a 64 page document, and is available to dowload by clicking here (1.87Mb).

3.      The letter sent back to you outlining your financial status, from your pension provider. This letter can be obtained by completing the Assets Assessment Application Form (Click here to download) and sending to your pension provider.

Fees
The fees for residential care vary, and are dependent on the outcome of your assests assessment. However, if you would like more information on the costs and fees, please visit the Department of Health and Ageing website.

Independent Living

Community Care

UnitingCare Ageing offers a diverse range of services. Please use the Advanced Services Search to find out what services are available in your area.

Community Aged Care Package

To be eligible to receive this package, your needs must first be assessed by an Aged Care Assessment Team (ACAT) member, to determine the assistance you may require, and if this type of package will satisfy your personal needs. However you are able to enquire about the service yourself, or through a family member or friend.

Home Nursing Service

The Home Nursing Service cares for a variety of people who include Department of Veterans' Affairs (DVA) Gold or White Card holders, the frail aged, the disabled, those with chronic illness, early discharges from hospital, or insurance recipients. To discuss your eligibility, please speak to a representative from your area. Where appropriate or required, a Doctor's Referral will be sought with your permission. However you are able to enquire about the service yourself, or through a family member or friend.

Extended Aged Care at Home

To be eligible to receive this package, your needs must first be assessed by an Aged Care Assessment Team (ACAT) member, and established as needing a high level of care. If EACH packages are available in your area, you have been assessed as requiring a high level of care, and expressed a preference to be cared for at home, then you may be offered an EACH package. However you are able to enquire about the service yourself, or through a family member or friend.

Extended Aged Care at Home - Dementia

To be eligible to receive this package, your needs must first be assessed by an Aged Care Assessment Team (ACAT) member, and established as needing a high level of care. You must also be experiencing symptoms and behaviours associated with dementia, which may be significantly impacting on your ability to live independently in the community. However you are able to enquire about the service yourself, or through a family member or friend.

 

Fees
Fees for CACP and EACH are charged on dependent on the number and type of services you require and also on your income and assets. The fee amount is agreed when you start on the service and is reviewed annually.

 

Aged Care Assessment Team

To be eligible for entry into any aged care service or facility and some of our community care programs you must first be assessed by your local Aged Care Assessment Team (ACATs).

Your doctor or local hospital can refer you to an ACAT. The ACAT is usually based at a hospital or community centre and may include a doctor, nurse, social worker, physiotherapist or occupational therapist.

ACATs can help you decide which services you need. They can help you to get residential care or a Community Aged Care Package (CACP), refer you to community care services, or help you arrange respite care in a residential setting.
