| Community Aged Care Packages (CACP) support older people who wish to remain at home. Each care package is personalised to your needs and can include the following services: • Meal preparation • Personal care • Light housework • Shopping • Transport to appointments • Outings • Respite for carers • Linking to other services, eg advocacy. Who can access the service? Any person who has been assessed as suitable by an Aged Care Assessment Team (ACAT). Your GP can refer you to the ACAT for your Local Government Area (LGA). Contact details are also available from your local public hospital. We currently provide Community Aged Care Packages in the Hornsby, Ku-ring-gai, Lane Cove, Manly, Mosman, North Sydney, Pittwater, Ryde, Waringah and Willoughby LGAs. We also have staff to provide care and support to the Chinese and Italian aged communities. |
