Select Care Packages are individually planned and coordinated packages of care, tailored to support older people who wish to remain at home.

These packages mean that you and your carer have control over the design and delivery of care and services you receive. You and your carer and family will make choices about the types of care services you use, the delivery of those services including who will deliver the services and when.

A low level Select Care Package can include the following services:
• Meal preparation
• Personal care
• Light housework
• Shopping
• Transport to appointments
• Outings
• Respite for carers
• Linking to other services and advocacy

A high level Select Care Package can include the following services:
• Registered nursing care
• Personal care
• Care by an allied health professional such as a physiotherapist or podiatrist
• Transport to appointments
• Social suport
• Home help
• Services which meet the specific needs of people with dementia who experience behaviours which may impact their daily quality of life.

To receive a UnitingCare Ageing Select Care Package, you must be assessed by an Aged Care Assessment Team (ACAT); live in the Northern Sydney Region and be eligible for a Community Aged Care Package (CACP), Extended Aged Care at Home (EACH) or Extended Aged Care at Home Dementia (EACH-D) package.
