- COMMUNITY AGED CARE PACKAGES (CACPs) A Community Aged Care Package (CACP) provides a range of services to assist older people to remain living in the comfort and familiarity of their home. Your case manager’s role is to coordinate services to meet your individual needs. For example, a package may give you help with personal care such as bathing and dressing, domestic assistance such as housework and shopping, meal preparation, medication assistance, or simply staying connected with your local community. - EXTENDED AGED CARE AT HOME(EACH) The Extended Aged Care at Home (EACH) program provides services to meet your advancing care needs through an individually tailored package to assist you to remain living in your own home for as long as possible, if this is what you prefer. An EACH package includes qualifi ed nursing input, and a range of fl exible services including personal care, social support and domestic assistance. - EXTENDED AGED CARE AT HOME - DEMENTIA (EACH D) Extended Aged Care at Home - Dementia (EACH D) is an extension of the EACH program (above) and is modifi ed to specifi cally meet the growing needs of people experiencing symptoms of dementia. - HOME NURSING SERVICES. The Home Nursing Service aims to contribute to improved quality of life and personal empowerment, maximising opportunities for positive experiences by providing clients and their families access to a range of services which refl ect their individual needs and circumstances. This is also echoed through the ensuring of fl exibility in service provisions, which will continue to meet the changing needs of the client, particularly in times of emergency. Nursing services include attending complex personal support needs, for example, wound care, diabetic care, medication administration, hand and foot care. The types of Home Nursing Services available include Private Home Nursing Services, and the Department of Veterans Affairs Home Nursing Program. - VETERANS HOME CARE. Veterans Home Care (VHC) aims to assist veterans and warwidows/widowers, to maintain optimal health, wellbeing and independence. As part of the program, you are encouraged to keep active and maintain social support networks with those who are impotant to you. The program provides a range of services to meet your needs including domestic assistance, personal care and respite care.
