Northern Sydney Wellness and Restorative Care Project


Following the 2008 Home and Community Care (HACC) National Forum, UnitingCare Ageing NSW.ACT was asked to lead a 12-month project looking into the opportunities for implementing Wellness, Capacity Building and Restorative care approaches within the Northern Sydney HACC sector.

Acacia Living Life is the pilot program, offering centre-based day programs in Berowra, Brooklyn and Galston. Acacia Living Life works in partnership with existing HACC and other service providers, voluntary groups, community care providers and church groups to meet the needs of local residents and their families.

In-Home Falls Prevention Project Report


UnitingCare Ageing Northern Sydney Region is pleased to release the final report of the In-Home Falls Prevention Project. Funded by the Australian Government, this Project has been implemented with 130+ Community Aged Care Package (CACP) clients in the Northern Sydney Region.

Utilising a case management approach to the prevention of falls in a high-risk population, the Project has resulted in a 35% reduction in reported falls as well as a reduction in falls resulting in admission to hospital.

Key features of the Project are:

  • Use of a validated screening tool
  • Targeted multi-factorial prevention strategies
  • Case manager support to implement strategies
  • Engagement of all levels of care staff as well as clients and carers
